Cystic parathyroid adenoma is a rare entity that should be considered in the presence of primary hyperparathyroidism (PHPT) with a cystic lesion within the area of the parathyroid glands. They represent less than 0.5% of parathyroid lesions and can be misdiagnosed with other cystic lesions.

A new publication in BMJ Case Rep, resulting from the collaboration of the Endocrinology (Dr. Andreia Fernandes and Dr. Joana Couto) and Head & Neck Surgery (Dr. Carlos E. Costa Almeida) Departments of Portuguese Oncology Institute of Coimbra (IPO Coimbra), is now available, highlighting the difficulties of diagnosing a cystic parathyroid adenoma responsible for a PHPT in a female patient. Besides the location and surgical approach of the lesion, the interest of this case lies in the misdiagnosis as an esophageal cyst. The strong clinical suspicion, combined with the Choline PET/CT, was paramount for the surgical decision.
Rare entities exist and must be taken into consideration. Knowing about rarities is the only way to diagnose them. Those who only have knowledge of the common entities will make many mistakes. Ignorance is audacious.
