Hepatic adenomas in male patients are always an indication for surgical resection due to the potential risk of malignancy. Hepatic lesions within peripheral segments (2, 3, 4b, 5, and 6) are suitable for laparoscopic resection. Minimally invasive surgery has many advantages over open surgery: less postoperative pain, faster recovery, lower risk of incisional hernias. With all surgical devices available, laparoscopic hepatic surgery is safe and feasible.
I present a video of a laparoscopic left lateral sectionectomy performed in a male patient diagnosed with a 3 cm adenoma within segments 2 and 3 of the liver. Parenchymal transection was conducted with LigaSure® Maryland and Cavitron Ultrasonic Surgical Aspirator (CUSA®). Segments 2 and 3 pedicles and the left hepatic vein were ligated with Signia® Stapling System EndoGIA (vascular cartridge - gold).
There were no intraoperative nor postoperative complications. Blood loss was minimal and surgery took 120 min to perform. The patient was very happy with the result, and he was discharged home on postoperative day 3. Laparoscopy is great for liver surgery.
